ELDER CARE COORDINATOR

JOB SUMMARY:

The Elder Care Coordinator oversees the coordination of the client’s health and long-term care while serving as their advocate and empowering them and their family to make informed decisions about their long-term care plan. The Elder Care Coordinator helps clients and their families navigate issues associated with aging, in addition to helping them maintain quality of life and well-being while accessing the appropriate level of care.

POSITION PURPOSE:

To work alongside the Elder Law Attorney and Paralegal Case Manager to develop a Life Care Plan that helps to meet the client’s legal, financial, and care needs. Also to serve as the primary point of contact within the law firm for care issues and help to help the firm develop our business relationships within the elder care community.

ESSENTIAL FUNCTIONS:

Responsibilities include, but are not limited to:

Client Relations

  • Provide comprehensive needs assessment including mental, physical, functional, and environmental to determine appropriate referrals and identify care strategies.
  • Travel to meet with clients in their homes, long-term care facilities, and hospitals. 
  • Assist with the creation and compilation of a Life Care Plan for the client, which summarizes their legal, financial, and care needs.
  • Document assessments, visits, and telephone calls with clients and community resources; compose correspondence to clients.
  • Help the client and their family arrange care services and identify medical equipment or supplies that can maximize care and quality of life whether inside the home or in a long-term care facility.
  • Counsel the family on care questions and concerns.
  • Counsel clients on questions they should ask of their medical team, care team, and attend care conferences, as needed.
  • Provide support, guidance, and advocacy during care crises.
  • Serve as an ongoing resource to the client and their families by answering questions, visiting the client, and periodically reviewing their care plan based on changes in circumstances.

Community Relations and Marketing

  • Stay current on local care options in the community, including the variety of options and the option’s reputation for quality.
  • Represent the firm to the healthcare and long-term care community through frequent visits. Create a positive relationship with management staff for purposes of client care maximization and referrals.
  • Participate in professional associations, both to enhance skills and to communicate the firm’s services.
  • Educate the community about the firm’s services and service model through speaking engagements, articles, and networking.

This position may be for you if you have the following:

  • Licensed Social Worker (LSW), Licensed Clinical Social Worker (LCSW), Master of Social Work (MSW), Registered Nurse (RN), or Licensed Practical Nurse (LPN) preferred.
  • Minimum of three to five years geriatric case management, geriatric care, and or geriatric social work experience.
  • Knowledge of Medicare/Medicaid preferred.
  • Comprehensive knowledge of the long-term care system and senior housing options.
  • Outstanding interpersonal skills.
  • Maintain current professional licensure.
  • Excellent written and verbal communications skills.
  • Extensive travel within the service area required.Maintain a valid driver’s license, good driving record, and access to a fully insured vehicle.

Job Type: Full-time
Expected hours: 40 per week.
